ACL injuries what are they and how to rehabilitate your knee

Anatomy of the ACL

The ACL is one of the four major ligaments in the knee joint and plays a crucial role in stabilising the knee during movements such as running, jumping, and cutting. It runs diagonally in the middle of the knee, connecting the femur (thighbone) to the tibia (shinbone). The ACL helps prevent excessive forward movement and rotation of the tibia relative to the femur.

Causes of ACL Injuries: ACL injuries often occur during sports or activities that involve sudden stops, changes in direction, or direct blows to the knee. Common causes include:

  • Non-contact injuries: These typically occur during rapid deceleration or sudden changes in direction, such as pivoting or landing from a jump.

  • Contact injuries: Direct impact or collision with another player or object can also result in ACL tears.

  • Overuse injuries: Repeated stress on the ACL over time, especially in sports that involve jumping and cutting motions, can lead to gradual wear and tear and eventual injury.  

Symptoms of ACL Injuries

The signs and symptoms of an ACL injury may vary depending on the severity of the injury but commonly include:

  • Sudden onset of pain in the knee

  • Swelling and stiffness

  • Instability or a feeling of the knee giving way

  • Audible "popping" sound at the time of injury

  • Difficulty bearing weight on the affected leg  Diagnosis and Treatment

  • Surgical intervention: Complete tears or significant instability often require surgical reconstruction of the ACL. This procedure involves replacing the torn ligament with a graft, typically from the patient's own hamstring tendon or patellar tendon.

  • Rehabilitation: Regardless of the treatment approach, rehabilitation plays a crucial role in restoring knee function, strength, and stability. Physical therapy exercises focused on strengthening the muscles surrounding the knee, improving range of motion, and proprioceptive training are essential for a successful recovery.
